1. Need Receipt Paper or kahit yung sa Shopee.
2. Need Details ng Modem.
3. Email ni Smart: sam@smart.com.ph
CC nyo kay NTC consumer consumer@ntc.gov.ph

Attache nyo nalang yung Receipt nyo, then serial number ng modem kasama pic ng modem.
