• Philhealth ID (Digitized/PVC or Laminated Cardboard with Barcode)
• Voter's ID
• TIN ID (Laminated)
• Government Service Insurance System (GSIS) e-Card
• Overseas Workers Welfare Administration (OWWA) ID/ OFW e-Card

For Filipino minors aged 7 to 17 years old

1. Primary ID (any of the following)
   • School ID (issued within the current school year and signed by the school principal or head of the educational institution)
   • Passport
   • Driver's License
   • Postal ID
   • Philippine National Identity Card
2. Parent/ Guardian's Valid ID (based on accepted primary IDs)
3. Signed Parental Consent form
4. PSA/NSO Birth Certificate
5. Proof of legal guardianship (if applicable)
