❓ Help I finally unblocked my Sim after 2 years

A. Copy of any VALID Government ID of the Subscriber (JPG/JPEG Format)
B. Selfie of the Subscriber holding the VALID Government ID
C. Clarification Letter with Date and Signature of the Subscriber

D. Copy of the SIM Card (Concerned Mobile Number)


Na blocked sim ko dahil rin sa vpn almost 2 years hindi ko nagamit ung data services nya at ung tm ang gamit ko nung na blocked ung smart sim ko.

Mag email lang kayo dito SmartFM@smart.com.ph make sure na tama ung format kagaya ng nasa taas.

Number ng smart nyo simula sa "9" walang zero
Then mag attach lang kayo ng Valid id
tapos picturan nyo ung sim card nyo with name and signature na nakasulat sa papel
mas maganda ung sim card na nablocked sa inyo eh registered at may signal para mag match sa system nila

or kahit pic lang mismo ng government valid id niyo po? Nag send po kasi ako ng without selfie pero nakakatanggap ako ng nito sa spam.

thanks po working po,pero prior ko po gawin yan tumawag muna ako sa csr at sabi mag wait ako ng 1Hr,habang nag wait nag send ako sa email.sa wakas pagbalik ko ng sim card ko sa phone ko nagka data signal na,thanks
