Things you need
+Photoshop
+Visual Basic
+Little Knowledge of VB and PS

Steps
1.Make an Image of a Desired or any form you want in Photoshop
2.Open Visual Basic and Make Form Border to "None"
3.Use the Image that you designed in PS change your form image background
4.Change the Color of the Form Background to Dim Gray
5. Set the Transparency color to Dim Gray

Tadaaaa. you have now your own form
