Just go to a Globe Store and bring proof of ownership (sim packaging, receipt or the like) and a valid ID. You might be required to fill out an affidavit of loss to further strength you claim (if prepaid) and questioned on the sim's recent transaction to validate your ownership specially if you do not have enough proof. If postpaid, just bring a vaild ID.
