I think OP’s registered SIM card got lost so now he’s asking how to report the loss and unregister the SIM.

Punta ka dun sa SIM registration site then pasok mo yung lost number. It should recognize na registered yung SIM. I believe you can update/make changes then.

salamat Lodi upon trying to check in their site sim registration needed OTP since the number was already lost I cannot provide it na... cguro punta nalang ako sa smart center to report and provide an affidavit of loss.
