hello po nag try po ako gumawa ng survey form using codeigniter then yung gusto ko po mangyari is ma collect yung mga answers ng mga mag aanswer dito sa pamamagitan ng database di ko po kase alam ano ilalagay dito huhu pa help naman po



then eto naman po itsura ng form:
