2. Duly accomplished APL Form.
3. One photocopy and original LTO-accredited medical certificate.
4. Original and one photocopy of Practical Driving Course (PDC)
5. Original valid student permit.
6. For minors, a notarized parental or guardian's consent with a valid government ID.
