Hmmm you can search naman yan sa google by typing in "How to replace lost Philhealth ID" then need ata gumawa ka ng "Affidavit of Loss" (This from way back pa at di ko na alam ngayon ). Sabi sa article no need na daw ang affidavit but i'm not sure kaya it's better to search na lang, try also watch on YT. About sa fees eh hindi ko alam kung meron but i think libre siya kung karton lang unlike sa digitalized na id ng philhealth.

pwede humingi uli sa philhealth pero baka hingian ka ng affidavit of loss,walang bayad yung philheath ID
